As part of each patient’s health care team, a clinical pharmacist is available to meet one-on-one with the patient in a private setting. This service is covered by most insurance companies and is available on a sliding fee scale for those without insurance. Clinical pharmacists work with each patient and his/her providers to develop an individualized medication plan. Clinical pharmacists help patients manage their medications by addressing things like cost issues, total number of pills prescribed, side effects, drug interactions, and monitoring and dealing with multiple medical providers. Pharmacists are part of the patient’s medical team and help manage many chronic diseases, including: Diabetes, Hypertension, High Cholesterol, Asthma, Chronic Obstructive Pulmonary Disease (COPD), and Hepatitis C, as well as other conditions such as Anxiety and Depression. Any action items or issues are immediately addressed by the patient’s medical team. Everyone is eligible for Clinical Pharmacy services. Appointments are available with the pharmacist by calling the health center’s scheduling department.
Frequently Asked Questions
Who can use the pharmacy services at our health centers?
You must be an active patient of the health center to use pharmacy services.
Can North Lima Community Health Center Pharmacy fill my prescriptions from specialty providers?
Yes, as long as our health center referred you to that specialist and you have been seen by us within the past 12 months, we will coordinate your care.
How can I qualify for discounted medication in the pharmacy?
Patients who are uninsured or underinsured must apply for discounted services and medications by meeting with a financial counselor to determine discount level. You will be required to bring in documentation regarding income, household size and if you have applied for insurance or medical assistance. You will be given an expiration date for your discount. This process may need to be repeated throughout the year to continue to receive medications at a discounted price.
Is the Pharmacy sliding fee the same as other departments?
The pharmacy sliding scale is different than the medical sliding scale. Pharmacy has three levels that patients fall into according to income and household size. Each medication has its own price within those discount plans. Some medications have set pricing for everyone.
